The Southampton Social Prescriber network is comprised of the SO:Linked Community Navigators, Social Prescribers based within Southampton’s PCNs (Primary Care Networks/Doctors Surgeries) and representatives from the local Voluntary Sector and Southampton City Council.
The network meets once every two months to catch up, learn, provide peer support and share information about services within the city.
We also send out regular information to members of the network, including updates and training opportunities.
